This strongly suggests a hydrostatic basis for the edema although it is not immediately clear how the increased pressure is seen by the capillaries. Presumably the constriction of the small pulmonary arteries caused by the alveolar hypoxia is uneven, as first suggested by Hultgren (2) and, as a result, those capillaries that are not protected from the increased pulmonary artery pressure are exposed to an increased transmural pressure.
